ACCIDENT REPORT INFORMATION

Accident Reports are usually available after five (5) working days by the following means:

1.) Appear in Person : Monday through Friday 8:00 a.m. – 5:00 p.m. Excluding Holidays.

  • Check or Money Order for $1.00
  • Self-Addressed Stamped Envelope
  • Include Drivers Name, Date of Accident, Approximate Time & Location of Accident.

    Ozaukee Sheriff will not fax or e-mail any incident based records releases.

    Fees for Records Request

    Payment is due upon the receipt of the records. Fees in excess of $5.00 will require prepayment.  The Sheriff’s Office may take up to 10 days to respond to record request. Request for Photographs and Video Tapes may take a minimum of 14 days to reproduce.

    For Criminal History Records, you must complete a Criminal History Record Request form and mail it to:
